If the patient is a newborn, please know that the health card number provided by the hospital is only temporary and you must register with OHIP as soon as possible to obtain a valid permanent health card.

1. OHIP does not cover for all services provided in the office.

2. Some of these services include complete insurance medicals, travel advice, certain vaccines not covered by OHIP, transfer of medical records etc.

3. If you have private health insurance that covers these services, we will bill you for the service and provide you with an official receipt. You may use the receipt to claim the amount from your insurance provider.

1. We are a community partner with the TMU school of medicine. So, there will be learners from various levels (medical students, residents) present from time to time.

2. You have the right to request a learner not be present during the appointment. Though, we would appreciate your co-operation in training the next generation of medical staff.

3. Learners also have the responsibility to uphold all legal aspects of the patient/physician confidentiality.
